The Colony at Eden Prairie Care Suites are designed to provide a comfortable home environment for residents who need regular help with daily personal care, such as medication, meals, laundry, grooming and hygiene.

Our beautifully decorated rooms, each with a private bath, will offer maximum independence and dignity, along with the security of our 24 hour licensed staff.

 

A variety of apartment layouts enable you to choose the perfect setting for your personal belongings and mementoes. All units feature wall-to-wall carpeting, basic satellite television access, central air conditioning and kitchen facilities. Our residents can choose studio or one- or two-bedroom apartment homes, many of which boast fireplaces, full-size washers and dryers and high-speed Internet access.

The Colony at Eden Prairie's goal is to afford those residents who are experiencing memory loss with as much freedom and fulfillment as possible. Studio apartment homes are specially designed to help residents feel safe and comfortable in a home-like environment. Our own licensed staff, trained to understand the unique needs of the memory impaired, work with each resident and their family members to customize a personal plan to maximize both self-motivation and functional independence. Surrounded by their own belongings and able to participate in meaningful activities, residents are assured a life of dignity and choice.

 

Beautifully decorated private rooms, each with a private bath, will provide residents with an option to live in a home environment within our community while receiving the individualized nursing care they need.

Residents and families will love our comfortable living and dining area. Licensed staff is on duty around the clock, and we offer a 1:3 staff ratio to meet the residents needs 24 hours/day.

Rehabilitation

Floor Plans

The Colony at Eden Prairie Transitional Care Center specializes in rehabilitation, short-term care programs required following surgery, injury or illness. These include cardiac care, recovery from stroke, hip and knee replacement, CHF, diabetic management and wound care.

Our Transitional Care Center provides disabilities treatment so residents can learn how to function and live within their maximum physical and mental capabilities. For our residents' convenience, rehabilitative therapies are administered in-house by Fairview Rehab. The congenial rehab staff is highly-qualified and focused on each resident's circumstances and needs.
